Key Responsibilities: Provide compassionate care to diverse patients and their families Educate and triage individuals with various health problems Perform follow-up and check-ins with patients to monitor chronic conditions Required Qualifications: Active Compact Multi-state RN License Bi-lingual in Spanish Experience in care of adult, chronically ill patients Working knowledge of computers and electronic documentation systems Expert communicator over the telephone for health care guidance
