Responsibilities Oncology Clinical Pharmacist - Outpatient Infusion Position Summary: Bring your passion for oncology and patient care to the outpatient infusion center at Dignity Health Cancer Institute at St. Joseph’s Hospital and Medical Center. As the clinical pharmacist you will be part of a growing program on the forefront of cancer treatment. We are looking for someone that is a great communicator, detail oriented, self-motivated, and able to problem solve through critical thinking. The clinical pharmacist works collaboratively with physicians, nurses, and navigators, and other members of the care team. Cancer Institute pharmacists are viewed as medication experts who focus their time on supporting nursing staff in the infusion suite, offering patient education, and participating in clinical decision making. They review and create care guidelines, and utilize Cerner to assist with appropriate drug therapy selection, management, and monitoring. Principal Duties and Accountabilities: Provides clinical pharmacy services to the infusion pharmacy service line to reduce harm to patients and improve patient outcomes. Follows all regulatory, accreditation and hospital standards of care, including USP chapters and department policies. Reviews and assesses order sets, regimen templates, and order sentences to identify, prevent and resolve potential and actual drug related problems. Monitors the medication formulary and assists in reviewing and updating as needed. Prepares and presents material to the Pharmacy and Therapeutics committee. Assists financial and pharmacy staff with provision of information needed to obtain prior authorization of ordered medications. Cross-trained to work in the infusion pharmacy and retail pharmacy to assist with staffing as deemed necessary. Acts as preceptor for students and residents - approving research topics, providing guidance in research and providing feedback and counseling on topic discussions.
