We are hiring a team of case managers that will be working hands on with patients out on the field. WC Health is seeking a full time qualified Mental Health Case Manager to provide services to consumers with mental illness. Essential Functions: Assess consumers' strengths and needs Develop individualized service plans Complete services assessments Link consumers with other community resources Coordinate services including health care and monitoring sufficiency of services and goal attainment Collaborate with other professionals. Perform other work as required. Knowledge, Skills and Abilities: Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required. Interviewing, service planning, consultation, and public relations skills. Computer skills and the ability to type required. Position Type: Full-time Education and Experience: Bachelor's degree in Human Services or related field and a minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience. Experience with electronic medical records (Preferred). Additional Qualifications: Must have a good driving record that does not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years (Required).
